Every resident of North Oaks has access to a Health Care Navigator. At your direction, the Navigator will guide you to the right staff and services to help you reach your health and wellness goals, including finding the fitness and exercise activities that are right for you and making the proper food choices to keep you in optimal health. The goal of the Navigator is to provide as much support as you desire, assist you in maintaining overall wellness, and help you avoid admission to higher levels of health services.
The Navigator also acts as a liaison for residents, helping to schedule health and medical related appointments and services. If you have to go to the hospital for a procedure or due to illness, the Navigator can assist with coordinating transfers and paperwork.
Health Care Services to Meet Your Specific Needs
Should your health care needs change, the Navigator will provide you with assistance in securing the appropriate level of health services, whether provided in your independent living residence by qualified and approved home health caregivers or in our Autumn Ridge Health Center.
Assistance in Living provides limited in-apartment services for independent living residents who may need help with certain aspects of daily living. Such services might include wake-up or bed-time assistance, bathing, and medication management.
